The application process can seem overwhelming at the beginning, but all the documents are requirements for employment purposes. We are pleased and honored to have you join our team.
We will need copies of the followings for your employee’s file:
Copy of immunization records
- Hepatitis B Proof of Titers, Vaccination or Declination proof
- Flu Vaccination or Declination proof
- COVID Vaccination or Declination proof
- Proof of MMR
- Copy of TB (PPD Skin test) within one year OR
- Negative PPD (1 or 2 stepper) within the last year OR Negative CXR (chest Xray) within 5 years or whatever is acceptable from your client and meet the standard criteria
- Physical Exam within the last year (You can get this done at your local provider)
office, local clinic or occupational health centres such as Concentra Locations
Proof of current Licensure
- Current Licenses
- Current CPR / BLS Card (From AHA or American Red Cross)
- Current ACLS/PAL/NRP (If applicable)
- At least 2 references
- Copy of your driver’s license (If applicable)
- SS card will meet the I-9 Criteria OR proof that you are authorized to work in America.
